Everything below comes with Hamaspik Medicare Choice.
More of your monthly check stays yours. Here is why.
Less going out for your care. More coming back for everyday costs.

Hamaspik began in 1980, helping families look after someone who needed daily help at home. Home aides. Day programs. The everyday work of keeping people in their own homes and out of institutions.
Forty-six years later, the Hamaspik network serves families in more than 25 counties across New York State.
Hamaspik Medicare Choice is our health plan for New Yorkers who have both Medicare and Medicaid, built by people who had already spent decades doing the caring.
“This review is strictly for the support and efforts I received from Tammy Harrington, the Medicaid Specialist at Hamaspik Choice. She was relentless in her availability and diligent in ensuring that my family member’s services were not interrupted. Thank you for your time and patience.”
“I had a great experience with Hamaspik. Tammy was very informative and helped me to get my uncle’s Medicaid situation taken care of expeditiously. She responded to my phone calls and emails in a timely manner.”
“After reviewing several Medicaid services for my father, I selected Hamaspik. It was the best decision I could’ve made. The staff was so professional and so helpful. They really made a difficult process very easy.”
“I don’t normally give reviews, but I would like to acknowledge the Director of Member service Betsy. One of the reason my mom is still in this plan is because of her. Overall, the plan is good.”
“Hamaspik always has excellent customer service! I had the pleasure of speaking to Ashley Nelson from member services and she was absolutely amazing. I had called in for my grandmother and not only did she help me with the matter at hand but made everything much easier. Thank you so much!”
“I would like to thank Alina Idrovo my mothers intake coordinator, for making her experience pleasurable, easy and not stressful.”
“Always a pleasure dealing with them and anything needed is always taken care of right away. As a caregiver I could not ask for more. Jennifer, went over and above to help my mother with her dental. Thank you again Jennifer you are incredible.”
“Navigating the world of health care let alone assistance for an elderly parent is a labyrinth that mere mortals could not possibly be expected to understand. The staff at Hamaspik knows the path.”
“My mother has been a member with Hamaspik for 6 months now and I am so pleased with the service she has received from Casandra and Jennifer in transportation to Nicole who handles her supplies.”
“We have been using Hamaspik Choice for my family member for the two months and I am excited about all the benefits it provides. The staffs are very professional, knowledgeable, and proactive.”
If your question is not here, a person will answer the phone and go through it with you.
No. As a Fully Integrated Dual Eligible Special Needs Plan (HMO D-SNP), the plan fully integrates your Medicare and Medicaid benefits under a single plan.
Eligible members may receive up to $315 a month, up to $3,780 a year, on the OTC benefits card. The healthy food, produce, and utility portions are special supplemental benefits for the chronically ill (SSBCI) that not all members qualify for.
We check your doctors against the Hamaspik Medicare Choice network before anything changes, and we tell you exactly what we find.
Yes. Hamaspik Medicare Choice is a full plan, so it takes the place of your current coverage rather than adding to it. Because this plan is an HMO D-SNP Medicaid Advantage Plan, any change to or from it must be processed and approved by both the State of New York and CMS.
Because Hamaspik Medicare Choice is a HMO D-SNP, you must be entitled to both Medicare Parts A and B and full New York Medicaid, and you may enroll only during a Medicare enrollment period or a special election period for which you qualify. All initial eligibility determinations are made by New York's Conflict-Free Evaluation and Enrollment Center, which will refer you to the plan if you are found eligible.
Eligible members pay a $0 monthly premium, and finding out what you qualify for costs nothing. You must continue to pay your Medicare Part B premium unless it is paid by Medicaid or another third party.
Yes, but only with your parent's consent or a valid appointment of representative. We will speak with the enrollee or their authorized representative before discussing any personal information, and eligibility and enrollment are handled through New York's Conflict-Free Evaluation and Enrollment Center.
Eligible members may use the monthly allowance for OTC health products for the purchase of food or utilities, or both.
The benefit will be administered using a pre-loaded debit card, valid for purchase at plan approved retail locations. Any unused portion of the benefit each month does not carry over to the next month.
This benefits is part of a special supplemental program for the chronically ill. Not all members qualify. Eligibility will be based on available data from claims submitted by your provider(s) and your annual health risk assessment. Members will be eligible for these benefits if they have three (3) or more chronic conditions, as listed below.
† Paid from your benefits card. One allowance of up to $315 a month, up to $3,780 a year, covers all three.
** Healthy food, produce, and utility bills are special supplemental benefits for the chronically ill (SSBCI), available to members with certain qualifying chronic conditions.
Hamaspik Medicare Choice is a Dual Eligible Special Needs Plan (HMO D-SNP) with a Medicare contract and a contract with the New York State Medicaid program. Enrollment depends on contract renewal and on verification that you are entitled to both Medicare and medical assistance from a state plan under Medicaid.
The maximum OTC and SSBCI benefit is one shared allowance of up to $315 per month, or $3,780 annually, not a separate allowance for each category, and not all members qualify. You must continue to pay your Medicare Part B premium unless it is paid by Medicaid or another third party.
Benefits are available only to eligible members of Hamaspik Medicare Choice and may vary based on eligibility, medical necessity, and plan rules. Not all members qualify for all benefits. Other eligibility requirements, limitations, and restrictions may apply.
Submitting the eligibility form does not obligate you to enroll. A representative can explain available benefits, confirm eligibility requirements, and help you verify whether your doctors participate in the Hamaspik Medicare Choice network before you make an enrollment decision.
